Two save files for Survivors and Peacekeepers. Both saves are located just encountering Waltz for the second time and before you reach the car factory/Central Loop.

All AVAILABLE Quests Done – I couldn’t get the quest ‘Out of your league’ to trigger on the survivors save, possibly due to a glitch.
All challenges completed with Gold ranking
Made with the intent of the player dedicating themselves to one faction.
I have chosen to help Harkon (and promised to leave the city with him) in each save to allow the player to get the best ending possible if they wish to do so.
No glitches and/or exploits have been used. This includes Third-Party software.

I have collected as many collectibles as I found. I shouldn’t have missed any unique collectibles.
Maya’s music box was returned to her on the survivor save. It has been put into the players stash on the Peacekeeper’s save.
Unique valuables are located within the players stash.
Sufficient crafting materials.

The following items have been fully upgraded
Throwing Knives
UV Bar
Spark, Fling, Empowerment, Reinforcement (Weapon Modifiers)
Bandages
Toughness and Endurance (Boosters)

In order to get the base ending possible all you need to do on either save is
Give the VNC Tower to Frank
Don’t kill Harkon
Don’t let Lawan kill Harkon
Select the option to save Mia, Harkon will save Lawan
